New School PlacementPro User Request Form

Congratulations on completing Partner Training.  You are now ready to request your PlacementPro account.  Note that this form is for SCHOOL Users! You will need the 2 clues and completion code you saw in the training content.

Name(Required)
Enter this number if appropriate for contact purposes!
Your full email address will become your username for PlacementPro!
Enter the full name of your School, College, or University without the Program Name.
List any campus(es) where you will be managing student placements. This is very important for multi-campus schools.
State(Required)
Enter your Program(s) of Study or degree as appropriate. Examples include Practical Nursing, Nursing BSN, Occupational Therapy, Pharmacy, Paramedic, Dietetics, etc.
Educational Users have full access to PlacementPro to request placements AND manage clinical requirements for students and instructors and assign students/instructors to placements. School Data Managers may only manage clinical requirements. They may not assign students or faculty to placements.
Primary Users are the main point of contact and use PlacementPro most often. They receive automated mail from the system. Backup Users use the PlacementPro system occasionally to assist the Primary Users. They may or may not receive email from the system.
