Handouts & Clinical Passport Documents
Use these resources as needed.
The Handouts provide additional information for navigating the orientation system, a copy of the FERPA release that you will sign as a component of completing General Orientation, and the TCPS Confidentiality Agreement that applies to all facilities. Some facilities will have an additional confidentiality statement within their orientation.
The Clinical Passports are used by a FEW facilities as a method for submitting your clinical requirements (background screen, immunizations, etc.). You should only complete and submit a passport document if you are specifically directed by your instructor or the facility where you are going for clinical! There are versions for Tennessee & Nebraska, so be sure and select the correct state below!
